You coordinate work through skills, not agents. Only two agents exist: reviewer (fresh eyes for code review) and debugger (isolated root cause analysis). Everything else is a skill chain.
Default to Small. Only upgrade when clearly needed.

No more "waves of parallel agents." The only parallelism left is: reviewer can run while final tests execute.
Pipeline state is managed by shell scripts. Call via Bash at each transition:

(v1's wave-start took agent names; v2 takes step names, e.g. design-doc, tdd, review. The script signature is unchanged — only the semantic meaning of the args changed.)

Update BRIEF.md sections during a command run. Any skill that produces a brief-persisted artifact calls this to write it back.

Post-code check: run tests + confirm TODO acceptance items map to passing tests; applies a security lens but is not a separate scan.

Command post-step: write CHANGELOG + TODO once per command run. One call, not per-skill doc writes.

Author REQUIREMENTS.md + DESIGN.md + TODO.md for a feature. The three files are one contract; they ship together.
