Must
- plan.surface_templates に従う（指定がなければ howto）。
- decisions の根拠は evidence がある場合のみ添える（ないなら“不明”）。
- exceptions は独立セクションに集約。
- 冗長複製を避ける（参照で済ませる）。
Output
- markdown文章
- 末尾に「IR References（使用ID）」を付ける（D*, P*, V*, X*）
